Description
Combivir is an oral drug that is a combination of lamivudine Epivir and zidovudine Retrovir. It is used for the treatment of infections with the human immunodeficiency virus HIV. Lamivudine and zidovudine are in a class of drugs called reverse transcriptase inhibitors that also includes zalcitabine Hivid and didanosine Videx.

During infection with HIV the HIV virus multiplies within the bodys cells. The newly-formed viruses are released from the cells and then spread throughout the body where they infect other cells. In this manner the infection continually spreads to new uninfected cells that the body is continually producing and HIV infection is perpetuated. When producing new viruses the HIV virus must manufacture new DNA for each virus. Reverse transcriptase is the enzyme that the virus uses to form this new DNA. Lamivudine and zidovudine block the activity of reverse transcriptase and block the production of DNA and new viruses. Specifically lamivudine is converted within the body to its active form lamivudine triphosphate and zidovudine is converted to its active form zidovudine triphosphate. The active forms are similar to the naturally occurring deoxycytidine triphosphate and thymidine triphosphate respectively chemicals that are used by reverse transcriptase to make DNA. The reverse transcriptase uses lamivudine triphosphate and zidovudine triphosphate instead of the naturally-occurring deoxycytidine triphosphate and thymidine triphosphate for making DNA and these active forms of the drugs interfere with the activity of reverse transcriptase.

The effectiveness of either lamivudine or zidovudine when used alone may decrease as the HIV virus develops resistance to the effects of the individual drugs. By combining lamivudine and zidovudine it is more difficult for the HIV virus to develop resistance to therapy since it must develop resistance to both drugs. As a result Combivir is more effective than lamivudine or zidovudine alone. Combivir does not kill existing HIV virus and it is not a cure for HIV. Combivir was approved by the FDA in September 1997.

How to use
For adults and adolescents weighing more than or equal to 30 kg: The recommended dose is 1 Combivir [Lamivudine and Zidovudine] tablet twice daily.
For pediatric patients weighing more than or equal to 30 kg for whom a solid oral dosage form is appropriate: The recommended dose is 1 Combivir [Lamivudine and Zidovudine] tablet twice daily.
If the pediatric patient is unable to swallow liquid oral formulation should be prescribed.
Since Combivir [Lamivudine and Zidovudine] is a fixed dose combination tablet pediatric patients with less than 30 kg body weight or patients with kidney or liver problem should not be prescribed.
Side Effects
The most common Combivir [Lamivudine and Zidovudine] side effects indicated are headache nausea nausea as well as vomiting muscle pain or bone pain general ill feeling and fatigue malaise diarrhea cough nerve problem including unusual sensation like burning or tingling and insomnia.
Other common side effects are dizziness fever or chill abdominal pain loss of appetite stomach cramps joint pain and indigestion or heart burn.
Some rare but serious side effects are anemia frequent infections unusual bleeding or bruising symptoms of too much lactic acid in the blood lactic acidosis such as tiredness trouble breathing lightheadedness weight loss slow heart beat muscle pain abdominal pain and shortness of breath signs of liver problem such as jaundice dark urine and enlargement of liver alteration of fat distribution in the body depressions and signs of allergic reactions like unexplained rash hives swelling in nasal cavity and throat itching and wheezing.

Drug Interaction
There are certain drugs when used concurrently with Combivir [Lamivudine and Zidovudine] can cause drug interactions.
Atovaquone: Increases the level of Lamivudine and Zidovudine in the blood.
Doxorubicin: Makes Combivir less effective.
Fluconazole: Increases the level of Lamivudine and Zidovudine in the blood.
Ganciclovir: May increase your risk of Combivir side effects.
Interferon Medications: may increase your risk of Combivir side effects.
Methadone: Increases the level of Lamivudine and Zidovudine in the blood.
Nelfinavir: Decreases the level of Lamivudine and Zidovudine in the blood.
Probenecid: Increases the level of Lamivudine and Zidovudine in the blood.
Ribavirin: Increase the risk of serious side effects and may make Combivir less effective.
Rifampin: Decreases the level of Lamivudine and Zidovudine in the blood.
Rifapentine: Decreases the level of Lamivudine and Zidovudine in the blood.
Ritonavir: Decreases the level of Lamivudine and Zidovudine in the blood.
Stavudine: Makes Stavudine less effective.
Trimethoprim and Sulfamethoxazole: Increases the level of Lamivudine and Zidovudine in the blood.
Valganciclovir: May increase your risk of Combivir side effects.
Valproic Acid or Divalproex Sodium: Increases the level of Lamivudine and Zidovudine in the blood.
Zalcitabine: Makes Combivir and Zalcitabine both less effective.



PREGNANCY: Use of Combivir during pregnancy has not been adequately evaluated.
Uses
Combivir [Lamivudine and Zidovudine] is primarily used for the treatment of HIV and AIDS. It restricts HIV virus to multiply and also prevent HIV from infecting healthy cells in the body.
Storage
Combivir [Lamivudine and Zidovudine] should be stored at a temperature between 2°-30°C 36°-86°F.

Precaution
Combivir [Lamivudine and Zidovudine] medication can decrease the ability of bone marrow to produce blood cells which can lead to anemia frequent infections and bleeding. So a frequent blood counts check-up is necessary while administering this medication.
Obese patients or patients with liver disease have a higher risk to lactic acidosis and hepatic steatosis.
It is not safe to use during pregnancy.
Combivir [Lamivudine and Zidovudine] passes through breast milk. It is important to know that breast feeding is not recommended.
