	Description
	Dilantin is the brand name of the drug phenytoin sodium the active component of Dilantin. Dilantin [Phenytoin sodium] is available in a form of 100mg extended oral capsule. Dilantin [Phenytoin sodium] is commonly used for the treatment of epilepsy. Specifically it is used to treat complex partial seizures generalized tonic-clonic seizures and seizures occurring during or following neurosurgery. Epilepsy is a brain disorder which occurs when there are recurring or brief changes in brain’s electrical system works. These changes in brain activities can lead to seizures. Dilantin [Phenytoin sodium] acts by affecting sodium channel in the brain. It does not prevent abnormal brain activity from starting but prevents abnormal activities from spreading to other parts of the brain. This way it helps to control seizures.


	How to use
	Dilantin [Phenytoin sodium] dosage should be individualized to give the maximum benefits. Adult patients who have not received any previous treatment can start the dosage with 100mg Dilantin [Phenytoin sodium] extended oral capsule three times a day. The dosage can be adjusted to suit the individual. For adults the satisfactory maintenance dosage is 100mg 3 to 4 times a day. The dosage can de further increased up to 2 capsules three times a day if necessary. In case of adults if seizure is controlled by divided dosages of three 100mg of Dilantin [Phenytoin sodium] capsules can be altered with only one dose of 300mg of extended Phenytoin sodium capsule. For pediatric patients the dose recommended is 5mg per kg in a day which can be divided into 2 to 3 equal doses with a maximum of 300mg of Dilantin [Phenytoin sodium] in a day per individual. Daily required maintenance dose is 4 to 8mg per kg. Children 6 years and above and adolescents may require a minimum dosage of 300mg per day. Dilantin [Phenytoin sodium] can be taken with or without food. If there is an occurrence of stomach upset it should be taken with food.


	Side Effects
	The most common Dilantin [Phenytoin sodium] side effects are unusual eye movements muscle coordination problems slurred speech and confusion. Other side effects are dizziness insomnia nervousness twitching headaches nausea vomiting constipation larger or fuller lips problems with gums and excessive hair on body or face. Serious side effects indicated are unusual and uncontrollable facial or body movements worsening of seizures unusual behavior or thinking unusual sensations like numbness burning or tingling high blood sugar swollen or tender lymph nodes bleeding jaundice bending of penis suicidal thoughts or behavior and signs of allergic reactions.


	Drug Interaction
	There are many drugs which may increase or decrease the phenytoin level or which are affected by phenytoin with concomitant use of Dilantin [Phenytoin sodium]. The most common drug interactions and drugs responsible are Increased level of phenytoin: acute alcohol intake amiodarone chloramphenicol cimetidine diazepam dicumarol estrogens fluoxetine halothane salicylates sulfonamides and trazodone. Decreased level of phenytoin: chronic alcohol abuse carbamazepine reserpine and sucralfate. Both increased and decreased level of phenytoin: Phenobarbital sodium valproate and valproic acid. Efficacy of drugs impaired by Dilantin [Phenytoin sodium]: corticosteroids digitoxin doxycycline estrogens furosemide oral contraceptives paroxetine quinidine rifampin and vitamin D.


	Uses
	Certain types of epileptic seizures can be controlled by using Dilantin [Phenytoin sodium]. Dilantin [Phenytoin sodium] is specifically used for the treatment of generalized tonic-clonic seizures complex partial seizures and seizures during brain surgery. The off-label uses include nerve pain arrhythmias and migraines.


	Storage
	Dilantin [Phenytoin sodium] should be stored at a temperature between 20o-25oC 68o-77oF away from heat and moisture.


	Precaution
	Patients with liver disease or elderly patients or patients those who are very ill may show early signs of toxicity so precautions should be taken during the medication. Patients with allergic reactions stevens-johnson syndrome and toxic epidermal necrolysis should be cautious and discontinue the Dilantin [Phenytoin sodium] medication. Patients hypersensitive to phenytoin should be cautious. Dilantin [Phenytoin sodium] may increase the serum glucose level in diabetic patients. During pregnancy Dilantin [Phenytoin sodium] should not be used as it may cause birth defects in face and limbs of the new born baby.
