	Description
	Topamax TOPIRAMATE is an anticonvulsant drug used in the treatment of epileptic seizures. That is why anticonvulsants are often called as antiepileptic drugs or antiseizure drugs. Topamax is an oral medication comes in form of tablets and sprinkle capsules. Topamax tablets are available as 25mg 50mg 100mg and 200mg. Topamax sprinkle capsules are available as 15mg and 25mg which should be administered as whole or opened and sprinkled onto soft food. Topamax is constituted of an active ingredient known as Topiramate and other inactive ingredients. Topiramate is a sulfamate-substituted monosaccharide. Topiramate present in Topamax is quickly absorbed after the oral use. Almost 70 of the drug is excreted in urine as an unchanged drug. The remaining 30 is extensively metabolized by hydroxylation hydrolysis and glucuronidation. Topiramate present in Topamax enhances Gamma-Aminobutyric Acid or GABA-activated chloride channels which plays an important role in regulating neuronal excitability throughout the nervous system and also inhibits excitatory neurotransmission through actions on kainate and AMPA receptors. It also acts as a sodium channel blocker.


	How to use
	To avoid early adverse effect like cognitive dysfunction the initial dose of Topamax should be normally low and increased in slow steps. The initial recommended dosage is 25 to 50 mg of Topamax daily in 2 single doses. Recommended increments are 25 to 50 mg of Topamax every 1 or 2 weeks. For maintenance treatment common dosage is 100 to 200 mg daily. The maximum dosage recommended is 400 mg daily in divided doses. For children the initial dosage of Topamax should be 25mg or 1-3mg/kg daily to be taken at night for the first week. The dose can be increased at an interval of 1-2 weeks by 1-3mg/kg daily. Adequate fluid intake should be maintained to minimize the risk of kidney stones. Topamax can be administered with or without food.


	Side Effects
	Topamax when used for the treatment of bipolar disorder adverse effects like short term memory loss and word-finding difficulty has been reported. Side effects reported due to use of Topamax for the treatment of frequently recurring migraines are tiredness dizziness lowered sense of feeling in the skin pins and needles in the fingers and toes difficulty with language nausea diarrhea decrease in appetite indigestion drowsiness dry mouth weight loss insomnia anxiety difficulty with concentration forgetfulness depression mood swings change in taste and vision disorders. Psychosis overheating of body less sweating eye pain and blood clots are also reported due to the usage of Topamax for general treatment. Acute myopia and secondary angle closure glaucoma has also been reported by the patients taking Topamax regularly. Other serious side effects reported are osteoporosis in adults and children and rickets in children.


	Drug Interaction
	Co-administration of Topamax with other inhibitors of carbonic anhydrase may lead to an increased risk of kidney stones. Topamax combined with enzyme inductors increase the elimination of topiramate from the body. The dosage of Topamax has to be altered to have desired effect. When Topamax is co-administered with phenytoin topiramate increase the plasma-levels of phenytoin. Alcohol causes increased sedation or drowsiness. It will increase the risk of having a seizure. Disorders like Oligohydrosis and hyperthermia are reportedly aggrevated when topamax is co-administered with antimuscarinic drugs.


	Uses
	Topiramate present in Topamax is used for the treatment of epilepsy in both children and adult. It can also be used as antidepressant in many cases. In children Topamax is used for the treatment of a disorder that causes seizures and developmental delay known as Lennox-Gastaut syndrome. Topamax is also used to prevent migraines. Psychiatrists use Topamax to treat bipolar disorder. The drug Topamax has been reported for use in treatment of obesity especially to help in the reduction of binge eating. Topamax is used to treat Post-traumatic Stress Disorder. Medication of Topamax helps in prevention of Periventricular leukomalacia in preterm infants after an hypoxic-ischemic injury. Uses of Topamax include treatment of bulimia nervosa smoking cessation obsessive-compulsive disorder alcoholism neuropathic pain and pseudotumor cerebri.


	Storage
	Topamax should be stored at room temperature away from direct sunlight heat and moisture.
